REQUEST FOR FUNDS - to be used by TEACHERS and STAFF only
Please fill out this form if you would like to request funds from the PTO IN ADVANCE for a project or activity that would benefit Concord students. The PTO exec board will notify you once your request has been reviewed.
REIMBURSEMENT REQUESTS - to be used by Concord Parents and Staff
Please fill out this form for all expenses already incurred related to Concord events and programs for which you would like to be reimbursed. Allow 1-2 weeks for reimbursements to be processed.
