REIMBURSEMENT FORM

• Parents:  Allow 1-2 weeks for reimbursements to be processed.  You must submit your receipt. 

• Teachers: Please use this form to request funds from the PTO or email treasurer@concordelementarypto.org with a link to the items you are requesting PTO funds for.

Pay your Annual PTO Dues Online

 Student PTO Fees are $35 per student annually. Help us reach our goal of 100% participation.  Pay your dues during the Membership Toolkit directory verification process.
